Primary Residence Verification Form

FEMA will be charging Nepenthe $250 per owner as a surcharge on flood insurance. We will be passing this fee on to the homeowners that do not provide proof of residency to us. We recognize that this is an inconvenience, but this is a straight pass-through expense unless that proof of residency is in place. To avoid this fee:

  1. Download the Primary Residence Verification form
  2. Fill it out and sign it
  3. Upload the completed form here (right), or submit it in person at the Nepenthe Clubhouse.


Provide one of the following acceptable documents which we can photocopy:

  • Driver’s License
  • Automobile registration
  • Proof of insurance for a vehicle
  • Voter’s registration
  • Documents showing where children attend school
  • Homestead Tax Credit Form for primary residence
